Monitor Reports Online

Monitor reports can now be completed online and sent directly to FLA. In order to access the forms, you must have registered as an FLA member, rather than as a public viewer. Registration is easy and confidential through the Members tab on this site (clicking on any of the sub-tabs will bring up the registration screen), and the forms are self-explanatory. For those of you wishing to continue mailing or faxing your forms, downloadable copies of all three forms are still available under the Forms tab.
